From PlanetOut.com:

Proper Prom Etiquette for Dining Out:

1. Sit up straight, take small bites and chew with your mouth closed. If you can accomplish this, your table manners are better than those of most people.

2. Remember that the people you are dining with are more important than the food you're eating. Pay attention to them -- especially your hosts. (Doing this will also help you avoid missteps when you're in unfamiliar dining territory.)

3. Do one thing at a time -- for instance, swallow a mouthful of food before taking a drink.

4. Employ that old bit of table-manners wisdom about utensil use: Start from the outside and work inward to your plate.

5. When you aren't eating, rest your hands in your lap or gently on the table; don't fidget or groom yourself (and keep your hands out of your hair).

6. Don't discuss food you dislike -- or graphically medical, overtly sexual or angrily political subjects -- while eating.

7. Use your napkin.

8. Be polite to and respectful of restaurant staffers.

9. No cell phones at the table.

10. Your bread plate is on the left side of your place setting; your glass is at the top right corner.
